About Transplant Check · In beta
Transplant Check is a Mental Wealth Solutions project being developed around kidney-care navigation for patients, caregivers, dialysis teams, nephrology practices, and transplant programs.
People can need a clear next question when care moves between dialysis, nephrology, and transplant teams.
The project explores ways to make stages, responsibilities, and practical questions easier to understand. A referral, evaluation, decision, and waitlist status remain separate events owned by the relevant care team or transplant program.
Patients and caregivers can use the educational pages and conversation guide to prepare questions. Care teams can see the handoff and barrier questions shaping development.
Those resources are editorial material. They are not a clinical assessment, referral intake, patient record, or transplant-program decision.

This site does not provide clinical care, assess eligibility, verify referral or waitlist status, receive referrals, match donors, or guarantee a CMS, clinical, or patient outcome.
Clinical and program decisions belong to the person’s care team and transplant program.
Official organizations and care teams remain the source of truth for decisions and status.
HRSA explains that transplant hospitals evaluate people and make program decisions. CMS defines and calculates its own measures. Transplant Check can explain these distinctions; it cannot speak for either organization or a specific program.
