Plain-language summary.
The current public site does not ask you to submit health information.
There is no sign-in, referral intake, patient account, contact form, payment service, advertising tracker, or analytics service on this beta site. The question guide runs in the page. Selections are not sent to a server or saved by the website.
Your browser, operating system, printer, or a PDF you choose to save may retain local copies. Manage those copies using the controls available on your device.
Data categories.
The site uses only what is needed to render the page and remember display preferences during the browser session.

- No health-information form or clinical record exists on this public site.

No health-data collection without approved infrastructure.
A future product or service would need a separate, clear explanation before it asked for personal or health information.
Any future handling of health information would require approved privacy, security, retention, vendor, and operational controls, along with terms that explain who receives the information and why. Nothing on this page represents those future capabilities as available now.
Retention, rights, and contact.
This public beta has no submitted health-data record for the site to retain or retrieve.
An approved public privacy contact path will be published before this site asks for personal or health information. Do not send medical records, diagnoses, insurance details, or other sensitive information through an unapproved channel.

We will post a new effective date and concise change note when the data boundary, vendor use, or public interaction changes.

Educational information cannot determine what applies to you.
Transplant Check does not assess transplant eligibility, check waiting-list records, recommend a center, provide treatment, or replace instructions from your care team. Ask the relevant team or transplant program about your circumstances and status.
